This is a YouTube video titled "What type of breast implants do you recommend and where should they be placed?" containing an informative monologue from February 23, 2018, discussing breast augmentation options including implant shape and placement. The speaker explains the pros and cons of round versus teardrop implants and the benefits of placing implants below the chest muscle for better aesthetic outcomes and reduced risks.
For breast augmentation, there are several different decisions that need to be made. Two of these decisions would include the type of implant, whether it will be a round or teardrop shape, and whether the implant will be placed above or below the chest muscle or pec muscle. In my practice, I choose to use mostly round implants for my cosmetic patients. The reason for this is that in my experience, I have not been able to see a significant difference between patients who have had round and anatomic implants. However, even though there's not very much of a difference, there is an additional risk of having the teardrop implants, which is that they can rotate or twist. If this happens, it results in an abnormal shape to the breast which needs to be corrected with additional surgery. With regards to placing the implants above or below the muscle, generally I recommend putting it below the muscle, particularly for patients who are thinner or have less breast tissue. The reason for this is that the muscle results in some additional protection and softening over top of the implant. This results in a softer shape, including the takeoff at the top of the chest, as well as a smoother outline to the implant. Additionally, there is some reduction in the risk around the implant, such as reducing the risk of capsular contracture or implant infection. The this occurs under the muscle, but the effect is lost if the implants are placed on top of the muscle.
