73% Of Pain Research Ignores This One Thing | #558

In this episode of The Evidence‑Based Chiropractor, Dr. Jeff Langmaid reviews a recent scoping review titled “Global Research Priorities for Back Pain, Neck Pain, and Osteoarthritis.” The review synthesized 23 research‑priority sets (361 unique priorities) from 18 studies to assess where the musculoskeletal research community is focusing its efforts. The key finding is that 73% of all identified priorities target clinical and basic science topics (diagnosis, treatment, mechanisms), while only about 11% address public‑health, health‑policy, health‑economics, or epidemiology. Patient input appears in roughly 70% of priority sets, but policymakers are included in only 17% (zero for back/neck pain). The priorities are overwhelmingly generated by high‑income countries, and virtually none of the priority sets include implementation plans or timelines. Dr. Langmaid interprets these gaps as both a problem and a massive opportunity for chiropractors to lead research in under‑explored areas such as health‑system integration, early conservative care pathways, cost‑effectiveness, and digital health. He emphasizes that without addressing these systemic questions, clinical advances will have limited impact on global disability from musculoskeletal conditions. The episode also includes sponsor messages and calls to action for listeners.
