In this video, surgeon‑narrator documents a body‑sculpting case targeting a male patient’s "buffalo hump" – an unwanted posterior fat pad that interferes with clothing fit and comfort. The patient previously underwent a liposuction attempt in another state two years earlier with no noticeable improvement, likely due to insufficient aggressiveness. The surgeon begins by establishing multiple‑layer local anesthesia (tumescent technique) to numb the area and help break down fibrofatty tissue. A laser is then applied to liquefy the fibrofatty matrix, facilitating its removal. Using a powered precision‑liposuction device, the surgeon extracts the tissue layer‑by‑layer, emphasizing thoroughness and control. The procedure is performed entirely awake under local anesthesia. The final result is a flat, depression‑shaped back with no residual fat, meeting the patient’s aesthetic goal. Post‑procedure, the patient is fitted with a compression pad and vest to be worn for two days, after which normal showering and activity resume. The video combines procedural narration, patient interaction, and visual documentation of each step.

Today's been another day of body sculpting. So we did laser liposuction, precision sculpting of the arms. We have a buffalo hump coming up next. Stay tuned. So our next case is a body sculpting case, the buffalo hump. It's in the back right here where people get extra fat pad here. They don't like it in their shirts. They just really are uncomfortable. All right. Hey. So this is our patient. You can see this hump. It's not huge, but it's there and it bothers the heck out of him, especially the appearance in clothes. And just to orient you, you know, his face is here, back of his head's here. He's actually had this addressed before in another state by another surgeon. It was 2 years ago that he had this addressed with a liposuction technique. He noticed no change, but what I find is that people just aren't aggressive enough and don't have the experience. So we're gonna take— So this is the other view turned looking to the left, and again, you can see this hump. We're gonna bring this in. So this is not the most impressive view, but you can see his previous incision from 2 years ago is right there. We are going to be working on all of this area, bringing that in. So we're getting started here. This is the hump we're going to take down. We're just putting a little numbing medication in where our access site is. We're now numbing him. We're going to lay down a lot of numbing in multiple layers here. Helps also break up the fibrofatty tissue. Pain medication in place. We place a lot in here. This allows for comfort but also allows me to get everything. You feeling okay? I'm feeling good. All right, moved on to the lasering part. So we're breaking up all this fibrofatty tissue, getting it more liquefied so we can get it all out. I spend a lot of time breaking this up so that way I can get everything, and he's very satisfied in the end. We've started our sculpting part now, so we are working through all this fibrofatty tissue, taking it out layer by layer. What makes my approach unique and more successful is that we use not only the laser and lots of numbing, but this powered device which really allows me to get through the tissue thoroughly. We have finished. It looks great. It's nice and flat. We've got a good depression right here, which is what he's looking for. It's really nice. No fat left. So we're all done with our buffalo hump reduction. It's done awake under local anesthesia with laser and precision-powered liposuction. So now that we're done, he's being placed in compression. There's gonna be a pad here and a vest that he wears to hold that in place for 2 days straight, and then he gets to shower and clean himself up, and he's gonna love it right off the bat.
