In this Romanian‑language health video, Mihail Pautov explains the condition commonly called “bila leneșă” (lazy gallbladder), medically known as biliary dyskinesia or functional gallbladder disorder. He describes how bile is produced by the liver, stored and concentrated in the gallbladder, and released by the hormone cholecystokinin after a fatty meal. Dyskinesia occurs when the coordination between gallbladder contraction and sphincter of Oddi relaxation is disturbed. Two clinical patterns are presented: a hypotonic form (incomplete, weak emptying) and a hypertonic form (spastic, painful contractions). The video walks through the Rome IV criteria that define biliary colic – right‑upper‑quadrant pain lasting ≥30 minutes, triggered by fatty meals, not relieved by position change or antacids, and without gallstones on imaging. Diagnostic work‑up includes normal liver/pancreatic enzymes, fasting ultrasound to rule out stones, and functional tests such as HIDA scintigraphy (ejection fraction <40 % indicates hypokinetic dyskinesia) or the “prânzul boidon” ultrasound after a fatty test meal. Risk factors are highlighted, especially female sex, estrogen‑driven cholesterol concentration, and pregnancy‑related smooth‑muscle relaxation. A thorough differential diagnosis list is given (functional dyspepsia, gastritis, ulcer, GERD, IBS, pancreatitis, hiatal hernia, biliary colic, gallstones). Management focuses on evidence‑based lifestyle changes – small, regular meals with modest fat, weight control, moderate exercise, stress reduction – and pharmacologic/herbal options. Choleretics (e.g., artichoke, silymarin) increase bile production, while colecistokinetics (antispasmodics such as drotaverine) improve gallbladder emptying; certain herbs (rostopasca, sunătoare) are warned against due to hepatotoxicity or drug interactions. Surgery (laparoscopic cholecystectomy) is reserved for refractory cases after ≥3 months of optimal medical therapy, severe pain, and documented low ejection fraction. Red‑flag symptoms that require urgent care (jaundice, fever, markedly elevated enzymes, persistent vomiting, weight loss, melena) are also listed. The video ends with a call for viewer comments on related topics.
