I have a process for how to handle when mistakes happen in my business, and it looks something like the chart you see above. It starts off with when the mistake actually happens, and then, depending on how you find out about the issue, how far along the issue is, you work through a bunch of different steps to make sure that the issue is not only resolved, but also finished by having a corrective action. Corrective actions can be small, like updating a step in an sop, or they can be big, like changing who is doing a process or adding a quality assurance.
