Welcome to your Tiny Health Pro Kit Sample Collection Video when you open your kit, the first thing you'll see is the instruction manual. Please keep it nearby for reference as you go through the steps. Be sure to keep your cardboard box because you'll need it later to return your samples. Inside your kit, you'll find a dry swab collection tube, a white capped vial with an attached spoon, a stool collector, disposable gloves, a polar gel pack, a FedEx clinical pack with prepaid return label, and a requisition form. Before you begin, fill out the requisition form completely, sign it and set it aside. And don't forget to activate your kit id online@tinyhealth.com activate the next time you're ready to use your restroom, take your kit with you and begin by washing your hands thoroughly with soap and water for at least 20 seconds. Next, open the stool collector and place it securely on your toilet seat. Be careful to avoid contamination with urine or toilet water. Grab your first test the swab test tube. Twist the tube to open. You only need to grab just enough so we can see some color on the swab. Once you've collected your stool sample, place the swab back into the tube until you hear a click sound. This is important to make sure that your sample is preserved. The swab should be at room temperature and stored away from direct sunlight. We recommend putting this back into your box. Do not freeze the swab. Next, take the second test, which is the vial with the white cap. Unscrew the cap and use the attached spoon to transfer small portions of the stool from different areas into the vial. Continue adding until you reach the fill line. Please do not underfill or overfill. When finished, screw the cap on tightly. Write down your name, date of birth and date of collection on the vial. Place the vial into the biohazard bag with the gel pack, seal it and freeze it overnight. When you're done collecting, simply flush the stool collector. After 24 hours, you can pack up your samples. To return your samples, place the requisition form back into the biohazard plastic bag, seal it and place the unfrozen swab and the biohazard bag containing the frozen vial, gel pack and the requisition form into the cardboard box. Important Ship Only between Monday and Thursday. If you're in the US you can schedule a FedEx pickup by calling 1-800-463-3333. You can leave your package outside your home for pickup or drop it off at a local FedEx store if you if you're in Canada, complete the airway bill and commercial invoice, place them in the pouch, and ship via FedEx pickup.
